New School Medication Authorization Form
At Sheffield City Schools, the health and safety of our students is always very important. As we get ready for the next school year, we want to share an important update from the Alabama State Department of Education with families.
A new School Medication Prescriber/Parent Authorization Form is now ready for any student who needs medicine during the school day for the 2026-2027 school year.
To ensure your child's care is not delayed, we ask parents and guardians to complete the new form with their child's doctor or healthcare provider and to schedule an appointment with the school nurse before school starts.
Please remember:
The doctor or healthcare provider and the parent/guardian must both fill out and sign the authorization form.
Prescription medicines must be labeled correctly. The label must match the information on the Prescriber/Parent Authorization Form.
